Leeds Intensive Family Support Services provides intensive support for families with young persons with complex mental health issues, learning disabilities, and ASD.

Our aim is to promote recovery, well-being, reduce relapse, and minimize hospital  readmissions. At LIFSS we assess and provide bespoke support packages that are responsive to the particular needs of a family and young persons living with the aforementioned conditions.

We receive referrals via Social Workers, directly from hospitals, schools, etc. Our Assessment team visits the family and the young person at home to do the assessment.

Based on the needs identified, a bespoke care/support package will be developed for a minimum period of three months. This will be reviewed midway in the three months with families and commissioners.  All necessary changes following this review will be included in the package/support plan.

We use an approach called person-centred planning, which puts the people we support at the heart of their support. We use a variety of tools to make sure people can exercise choice and control in their lives. Through person-centred reviews, we look at what’s working and not working with your relative’s support, working together to decide how things can improve. We use these reviews to update your relative’s support plan with new developments, goals and needs.

All LIFSS support teams are trained to understand and meet the individual needs of the person they support through a support plan co-produced with family and wider circles of support.

Driven by our values, we place the people we support and their families at the heart of everything we do. We want every person we support to have a great life, with excellent outcomes.

We know how tiring it can be to care for somebody around the clock, and that it can be hard to entrust the care of that person to others. That is why LIFSS offers personalised respite care for disabled adults and their families.
